EAC G7 2025 Permission Form

PLEASE READ THIS FIRST

G7 is Engadine Anglican Church's annual youth camp and is a great time for youth to develop and strengthen friendships, ask the big questions in life and consider what the bible teaches together. This year the camp has the theme "Tradies" and we'll be looking into the book of Haggai together with Daniel Bishop as our speaker.

 

All of our leaders are required to undergo a Safe Ministry check, Safe Ministry training and a Working with Children's Check before participating in our ministries.

Dates: Saturday 12th of April 2025 at 11:00 AM to Wednesday 16th of April 2025 4pm (approximately). 

Pick up and drop off for camp is at Engadine Anglican church.

Location of camp: Anglican Youthworks Waterslea campsite
439 Koloona Dr, West Cambewarra, NSW 2540

We will provide a bus that will transport the youth from the Engadine Anglican Church to the campsite. We also often have a few more youths than can fit on the bus, in this case, some youth will travel with leaders who are Green P's and above. If your child may be travelling by car we will contact you to discuss it beforehand.

Please note that all communication will be sent to the email address you provide for the Primary contact person in the registration process.

IMPORTANT: The way our registration process works is that you register using this form and then secondly pay directly to our church accounting system so that you do not incur any additional transaction fees. This means you will be required to pay after you have registered here.

Please note the payment directions below so that you can pay after you have registered your child/children.

REGISTRATION CLOSES SUNDAY MARCH 16th AT MIDNIGHT. Please register before then. If you have any payment inquiries or need financial support please contact our youth minister Daniel Bishop as soon as possible as we have funds to support those in need of financial aid.

Youth - $500 per camper  
Leaders - $450 per leader

Unfortunately due to the cost of living increasing the campsite fees have increased significantly this year to match the increase. In order to manage this cost increase we have reduced camp by a day.

If you are unable to pay the registration fee, please do not let that stop your youth from coming. Please contact our youth minister Dan on 0410 830 052 if assistance is required. We can create pay-over-time plans and there is money set aside for aiding those unable to pay as well.

Health and medical information

Please provide details for all of your children's health/medical information. In the "give details" sections please specify the names the details you provide are applicable too.

e.g:
 Daniel has Asthma - Ventolin to be carried with Daniel at all times.

IF THE CAMPER REQUIRES MEDICATION ADMINISTERED

Please note that only prescribed medication will be administered during the duration of the camp (including Roaccutane).

All medication must be clearly labelled with a pharmacy label containing the camper's full name, dosage, doctor's name and expiry date.

All medication will be held and administered by an assigned Camp Medical Leader unless advised by parents except ventolin and EpiPens. If you wish for your youth to administer their medication please input that request and information below.

Current Anaphylaxis and/or Asthma Action Plans are to be provided via email to youth@eac.org.au before the rego form closes.

Photo's and footage taken during activities on camp may be used for promotional material on the social media pages of Engadine and Heathcote Anglican Churches. If there is a particular reason that photo's which include your child cannot be used in this way please email youth@eac.org.au with a photo of your child attached so that we can ensure they are not placed on our promotional material.

INDEMNITY
By selecting "YES" in the box below I am indicating that I am the parent/guardian of the child/ren mentioned in this form and that I am willing to permit my child/ren to participate fully in all activities associated with the camp, including (but not necessarily limited to) those indicated in the program. While every precaution shall be taken to ensure the good welfare and protection of the applicant camper/s, Heathcote Engadine Anglican Church, their Council members, staff members, employees, or any person acting on their behalf are hereby released from any and all liability in the event of any accident or misfortune that may occur to the applicant camper/s or damage or loss to their property. In the case of a medical emergency, I hereby give permission to the Doctor chosen by the Camp Director to secure proper treatment for and/or order hospitalisation, injection, anaesthetic, or surgery for my child/ren as named. I understand that every effort will be made to contact me prior to instituting such procedures. I also give permission for my child/ren to be transported to camp either by bus or car, understanding that if by car I will be notified first and this will be discussed as an option.
